How the transition to home care works, step-by-step ...
Meet with an Amada Mesa Senior Care Advisor for a free in-home care needs consultation either for yourself or a loved one.
Receive a follow-up call from one of our schedulers to verify the dates and times for caregiving services on your calendar (or that of your loved one’s).
An experienced Care Coordinator on the Amada Mesa team will accompany your caregiver on the very first shift and personally introduce you or your loved to the caregiver. The primary focus of the Care Coordinator is to ensure a smooth transition when starting home care.
After the first shift is completed, our scheduler will reach out to you for your feedback on the caregiving services provided, how everything went and whether any adjustments need to be made.
Upon completion of the first week of in-home care, the Care Coordinator personally visits again to assess whether your or your loved one’s needs are being met. The Care Coordinator will visit a third time after 30 days of caregiving services to check again that the in-home care process is proceeding smoothly and meeting your family’s needs.
After a successful third visit, the Care Coordinator will schedule a future check-in date based on client needs. In addition, we monitor our caregivers and regularly check in with clients and their families to make certain that we are doing all we can to help our senior clients live safer and happier and more fulfilled lives.
