Advance Care Planning at Amada
At Amada Lehigh Valley, we offer a holistic approach to care management. Our program, led by highly experienced social workers, aims to help our clients navigate their care effectively, while also ensuring their families have peace of mind. At Amada, our Care Management services encompass the creation of tailored care plans, acting as the client’s advocate as needed and ensuring our client benefits from a wide range of care and activities that cater to mental, physical and emotional well-being.
The Amada Lehigh Valley Care Management program provides a variety of services, including but not limited to:
- Comprehensive care planning with ongoing support and assessments
- Client advocacy with regular updates on their status
- Ensuring medication compliance
- Providing transportation and help with errands
- Escorting clients to medical visits
- Assistance with handling mail and finances
- Expert guidance through the Long-Term Care insurance process
- Connecting clients with community resources as needed
- Support with transition to assistant living communities or placement in care facilities
- Regular check-ins for clients residing in residential communities and communicating updates to their families
- Management of emergency situations (urgent care, ER)
- Support for mental health, including counseling services
