What is GUIDE? GUIDE is a groundbreaking program prioritizing caregiver support to enhance the quality of life for dementia patients.

- Respite services
Our program will begin in July 2024 and evaluate based on care coordination, patient quality of life, caregiver burden, utilization, and long-term nursing home stay rates.
When family caregivers are involved and engaged in care plans, hospital readmissions decrease by 25%. These figures emphasize the pivotal role that caregiver support plays in enhancing overall health outcomes for patients.
Who is Eligible?
✔️ Traditional Medicare beneficiaries with a confirmed dementia diagnosis
(including Alzheimer’s, LBD, FTD, vascular dementia, etc.)
✔️ Living at home or in the community
