New Choices Waiver in Utah

Transition From a Facility Back to Living at Home

The Utah New Choices Waiver (NCW) is a Medicaid program that helps seniors and adults with disabilities transition out of nursing facilities and back into community living. The program provides funding for home and community-based services so individuals can safely live in their own home, with family members, or in assisted living rather than remaining in an institutional setting.

The goal of the New Choices Waiver is simple:  Help individuals regain independence while receiving the care and support they need in a community environment.

Who Qualifies for the New Choices Waiver

Individuals may qualify for the New Choices Waiver if they:

  • Live in Utah
  • Are 65 or older, or 18–64 with a qualifying disability
  • Currently live in a nursing facility or other qualifying institutional setting
  • Require a nursing-facility level of care
  • Meet Utah Medicaid financial eligibility requirements

Because the program has limited enrollment slots, eligible applicants may be placed on a waiting list until funding becomes available.


Services Covered Under the New Choices Waiver

Participants in the NCW program may receive a variety of services depending on their care needs. These services help individuals remain safe and independent while living outside of an institutional facility.

Common services include:

  • Personal care assistance
  • Help with bathing, dressing, and grooming
  • Homemaker and household support
  • Meal preparation and nutrition support
  • Respite care for family caregivers
  • Adult day services
  • Personal emergency response systems
  • Home accessibility modifications such as ramps or grab bars
  • Assistive technology and specialized equipment
  • Non-medical transportation

These services are coordinated through a case manager who develops an individualized care plan for each participant.


Where Participants Can Live

The New Choices Waiver allows individuals to move from institutional care into community settings such as:

  • Their own home
  • A family member’s home
  • Assisted living communities
  • Adult foster care or host home settings
  • Independent senior housing

This flexibility allows participants to live in the environment that best supports their independence and quality of life.
